This position requires expertise in case management, patient advocacy, and an ability to thrive in a fast-paced environment. Assess new patients, reviewing their medical history and planning appropriate care pathways. Collaborate with healthcare professionals to formulate interdisciplinary plans for patient care. Monitor patient progress and adjust care plans as necessary. Advocate for patients' needs and provide support and education to patients and their families. Ensure compliance with healthcare regulations and maintain accurate patient records. Strong clinical judgment Excellent interpersonal and communication skills Proficiency in healthcare software and systems Ability to manage multiple cases efficiently Strong organizational skills Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Valid RN license Certification in Case Management (CCM) preferred
